**As a private practice, we regret that we are not able to offer a sliding fee scale

We are on most local and regional insurance panels including, but not limited to:

  • Medicaid/Title 19

Employer insurance plans change frequently and we often join new panels. Please call to see whether we are a provider for your insurance or if you need more information.
We are also on many Employee Assistance Program (EAP) panels.

If we are not on a certain panel and there are no other available, local providers, we can help explore the possibility of obtaining off-panel authorization.
